How To Register

- Child Registration Form
- Child Information Form
- Child Health Form  (Child Heath Form needs to be filled out and signed by your child's pediatrician).

*If your child requires any type of prescribed medication, please fill out Child Medication Form.

Step 2 - Submit your finalized Registration Packet:
